Must have at least 6 months OTR driving experience in last 36 months, no more than 2 moving violations in 12 months, no more than 3 in 36 months, no more than 1 major violation in last 3 years, no major accident in 3 years, no more than 3 minor preventable incidents in 3 yrs AND no more than combination of 4 violations/accidents/incidents in 3 years. No DUI in 3 years, no multiple DUIs, no possession misdemeanor in 5 yrs, no major felonies ever.
